Helping to improve care for those living with dementia
Through our ongoing partnership with Alzheimer’s Society, our Quality & Clinical Standards Director / Superintendent Pharmacist Steve Howard has been invited to be a member of a Steering Committee looking at how we care for people living with dementia.
Funded by Alzheimer’s Society, the national project will look at developing new sustainable models of primary care-led, person-centred care across health and social care.
In England, post diagnostic dementia care is poorly coordinated with marked geographical differences. The Alzheimer’s Society report ‘Dementia 2015: aiming higher to transform lives’ found 49% of people with dementia received insufficient support after diagnosis. We hope this project will address these concerns.
The project addresses the 2016 World Alzheimer Report recommendations for future post diagnostic dementia care to be primary care-led ie people with dementia and their families receive ‘most of their care, most of the time’ from their GP. This should allow more appropriate use of expert resource and maintain better quality of care, whilst meeting the needs of a rapidly ageing population.
We believe the outcome will be a good practice model of primary care-led, post diagnostic dementia care that will be cost effective, person-centred and sustainable.
